due to current pressures we have to prioritise NHS work, as a result
Some services are not available on the NHS
the form can left in at reception for review and a decision made within 5 working days. please contact reception for an update if this period is exceeded.
For example:
- some Driving licence medicals
- some Travel cancellation forms
- some private medicals/ health declarations / disclaimer forms
A fee is payable in accordance with the standard BMA rate. A list is available at reception.
